About Hands of Hope, our pediatric program
We honor the individuality and humanity of every person living with dementia, especially as they approach the end of life. Our care is person centered, grounded in empathy, and tailored to support not just cognitive and physical needs, but also emotional and spiritual well-being.
Our care approach for a patient with dementia focuses on personhood and optimization of their function and quality of life. We promote Best Ability to Function (BATF) for the patient through ongoing assessment of emotional, physical, sensory, and cognitive capabilities and limitations.
We adapt communication methods, behavior de-escalation techniques, and safety interventions for the specific person in our care. We shift thought from what the patient can no longer do to focusing on what they can still do, and we use this framework to guide care delivery.
Hospice care is not only for the patient; it also supports the family. Caregiver stress can be compounded by the complexities of dementia care. Our team is there every step of the way. We offer education about person-centered care and the cognitive and functional challenges their loved one may experience, as well as guidance and support.
Our goal is to help create meaningful moments, have successful visits, and strengthen relationships with an understanding of the patient’s capabilities.
Our clinicians are Dementia Care Specialists credentialed through the Crisis Prevention Institute (CPI). The Dementia Capable Care model from CPI teaches clinicians and caregivers how to support individuals at every cognitive level and stage of dementia with tools to prevent and de-escalate distress behaviors without the use of chemical or physical restraints.
